How to reach us
New patients
If you’re new to Altacare, please use the “Book an online visit” button in the top menu or on this page. Through our OptiMantra portal, you’ll be able to:
- -Request your first online visit
- -Choose the type of service you need
- -Complete your intake forms securely before your appointment
Existing patients
If you’re already a patient, you can also use the “Book an online visit” button to go to the OptiMantra portal. Once logged in, you’ll be able to:
- -Book or change appointments
- -Send secure messages to our team
- -Complete any additional forms requested by your clinician

Altacare Telehealth is an online-only clinic. We do not see patients in person or accept walk-ins at a physical address. If you have a general question that does not include medical details, you may reach our office at:

Altacare Telehealth is not an emergency service. If you are experiencing chest pain, trouble breathing, signs of stroke, thoughts of harming yourself or others, or any other life-threatening symptoms, call 911 or go to the nearest emergency room immediately.
